Intumescent white cataracts pose the risk of expulsive anterior capsule tears during the capsulorrhexis (the “Argentinian Flag Sign”). Traditionally, the lens is decompressed with needle aspiration of liquid cortex to prevent this complication. In this video, Dr. Harris demonstrates an alternative decompression technique using the phaco probe. First, trypan and Healon 5 are injected, followed by a small amount of BSS above the capsule to prevent corneal wound burn. Next, the phaco probe pulses through the anterior capsule then aspirates the liquid cortex to normalize the intracapsular pressure. Finally, the anterior capsular defect is converted into a continuous circular capsulorrhexis.
